Thanks to the generous contributions of a group of dedicated and caring mental health advocates, we are pleased to announce that funding has been secured to support a limited number of consumer scholarships to the 2012 National Zarrow Mental Health Symposium and Mental Health America Annual Conference.

Who’s Eligible:
Scholarships are reserved for individuals living in recovery from mental illness and/or substance use (consumers).  In order for your application to be considered, a short letter of recommendation from a mental health advocacy organization or treatment provider must be submitted with your application. A maximum of 2 applicants per referring organization please.

Scholarships are awarded to the individual and are non-transferable. If you are selected to receive a scholarship and are not able to attend we request that you please notify the Mental Health Association in Tulsa no later than Friday, September 14th so that we have the opportunity to provide the scholarship to another deserving applicant.

Deadline to Apply:
Close of business Wednesday, September 12, 2012. Applications will be reviewed on a first come, first served basis. Recipients will be notified on or before Friday, September 14, 2012.

What’s Included:
Scholarship recipients will receive one full conference registration which includes access to all 3 days of the event along with the Thursday evening Clifford Beers Awards Dinner featuring Jessie Close.  Awardees will be responsible for all of their own travel and lodging related expenses, including meals outside of those served during the conference.

How to Apply:
Applicants must submit: 1) A completed scholarship application, 2) Personal statement regarding why they would like to attend/how they will use the knowledge gained, 3) Short letter of recommendation from a mental health advocacy organization or treatment provider.  Incomplete applications will not be considered.

Application materials may be faxed to the attention of Kimberly Cummings at the Mental Health Association in Tulsa at (918)585-1263. Applications and support materials may also be submitted electronically to kcummings@mhat.org.  Completed applications, including the requested support materials, must be received on or before Wednesday, September 12th.

Travel:
Scholarship recipients are responsible for their own travel related expenses, including transportation to and from the Tulsa Convention Center, hotel accommodations, and food and beverage outside of meals served at the conference.  For more information on travel, including special hotel rates for conference participants, see www.fromhousingtorecovery.org/travel.
